I do design in Eagle and will provide all files including BOM and Gerber files. As you have mentioned that your a Software Engineer and you have a working prototype, which means you have done the coding part. I wanted to be clear about this because I am not that good at coding. Regarding the pin configuration for the PCB, I won't mind sticking to the I/O you have used. I will surely suggest changes if I have better solutions. When you say that filtering will be required, what kind of filtering you mean? 12V input sounds good. All other components are straight forward; regarding the RJ45 jack, if there is enough space left we can add the jack.
