Salesforce - Duplicate Contact Identification and Merge

We need someone with expertise in Salesforce and data de-duplication/cleansing for the following:
1. Provide an Excel (or comparable) report showing all exact and near matches between contact records. Anything over a 60% match ratio should be reported.
An example match:
Contact 1 = John Smith / 1000 Paddington Road Chicago Illinois
Contact 2 = John M Smith / 1000 Paddington Rd. Chicago IL
2. Match comparison should take place on common fields such as email, name, address, phone, etc
3. Report should be sorted by matched records so matched duplicates appear together on consecutive rows.
4. Match ratio should be shown if possible (i.e. 88% match)
5. Report will be examined by us and we will identify the Master record that all of its duplicates should be merged into.
6. You will use this data to merge those records together in Salesforce (using Upsert or any other best practice you suggest)
7. Child objects within each Child record should be maintained. For example, in the above example for John Smith, if John Smith is the Master record and has 3 notes attached, and John M Smith is the Child record merging into the Master and John M Smith has 2 notes attached... those 2 notes should be added to the 3 in the Master record.
