Create a PDF for EVA mold and PDF for screen printing from a 3D model

We are a giftware business creating a new product in our glassware range and trying something new with our packaging. We have a 3D model of a bamboo box with EVA insert and the manufacturer requires a PDF for the printing on top as well as a PDF for the EVA mold inside the box. It is imperative that the PDF file be acceptable to our manufacturer to create a 3D EVA insert for our box - we will seek to provide as much information from our manufacturer as you need to complete the task. The box size is 310mm x 204mm x 120mm and I also have a PDF of the top printing requirements but it has measurements all over it and is not suitable for printing. For someone that knows what they are doing I am certain it is a simple task, we are looking for a quick turnaround on this job where possible.
