Design A User-Verification Page For A Web App

We are Athena Bitcoin, a Bitcoin ATM company based in Chicago, IL.
We are looking to have a page designed/styled for one of our current projects that will serve as a way for users to verify their ID (by uploading or scanning their license, passport, etc.) and to provide a bitcoin address to receive funds to.

The page will be separated via 4 or 5 different floating segments (rectangles) that each either provide instructions, or allow the user to enter/verify information (certain elements will also be hidden depending on which step the user is at in the process).
We need to have this page styled so that it looks more aesthetically pleasing, it is very basic right now. We're looking for a designer that can work with photoshop and also CSS in order to help us develop some of the front-end for the page.
Our website is [login to view URL], we would like for it to be on-brand but are also open to some creative liberties to be taken as we are also looking to eventually refresh and evolve our branding. The footer does not need to look like it appears in the screens, it can be short and just say "© Athena Bitcoin, 2019. All rights reserved."
