I need to add Logo, company details on top, after need to add fields like text box, numbers box, email box, address box, add file photo only that can use camera and scan, box for comment , box privacy and policy comment, check mark, drop down list, signature using touchscreen, add time and date an the file. And when this form will be submitted will be able to access in the back end management and use email or phone number or name or address or serial of the device to find the web form that was filled in the past .
