Acrobat form: Enable/Disable Radio Button Group with Checkbox state

I need some help with an Acrobat Form I have set up. See the attached file - There is a check box to ask if the question is applicable. Initially I would like the radio buttons for each question to be disabled and are only enabled when the "Applicable" check box is ticked. The user can then choose a radio button answer. If the checkbox is then deselected the the radio buttons for that question should be disabled and any value cleared - i.e no radio button are selected for that question. I need you to add the necessary scripts to this sample file and also write instructions to me how this was done. Thanks.
