1. We need ability to change what questions are asked based on answers. Based on the answer clicked, i need emails sent to different email addresses containing all the information from the form that was filled out. 2. Need the ability to change this form OURSELVES . So if I wanted to add a question or statement or even change who gets paged when that would be helpful. 3. Needs to be able to change what is done based on the time. During business hours will go to pager (email account like xxxxxxxxxx @ usamobility .net) but after hours certain trees will end up going to an email account (like xxx @[login to view URL]) 4. Looking for a full-stack developer (most of the work is back-end). 5. Details about what exactly needs to be done: Recreate the form we made but clean up front end to make it professional, allow back end to reliably send the email to correct pager based on answers, and allow same functionality which is the ability to change. 6. Any visual front-end content which we want to add to the page: Nothing complicated. Just not the usual google form look, and put our logo somewhere there.
