Create an interactive PDF

Job Description:
Will provide complete PDF outline. The idea is to get all items described for each page placed on each page with appropriate space for data entry, and to add data type checking for fields that require currency, numbers, text. Some minor calculated fields. The attached blueprint is the accurate flow of information, it just needs to be 'pretty' (without grid lines, properly formatted and sized text boxes/fields, etc.) on each page. There should be a SUBMIT button at the end of the document that sends a clean output PDF to a specific email address. Please see attached doc and REVIEW CAREFULLY.
