Survey program needed

Good Morning
I am looking to build a survey program which I will be able to edit, add, remove questions, view result right in my own control panel.
I have attached some examples of what I am looking for. The excel spreadsheet includes all the categories for the database that I need. I would also like those categories to be sortable for all the questions that I will be inputting. Then I need to be able to pick individual questions from that database and have it dumped into a survey format.
I have also attached an example of the survey format so you can see how that will be.
I am willing to pay up to $150.00, I need this done within 7 days.
