URGENT: 1 DAY PROJECT: Landing Page/Order Form Coding(repost)(repost)

I will be awarding this project TODAY (within just a couple hours of posting- as soon as the first bid I like comes in). I have a functioning landing page/order form process in place that I'd like you to modify (change out some graphics, make the form fields nicer looking, and add some video and new text). The pages are already live at: [login to view URL] Upon acceptance, you will take the new graphics I have and update the site design with them. All of the form field coding is functional, I'd just like the boxes to be a nicer look, like: [login to view URL] See how the boxes and form field text highlights and looks more Web2.0 than what we have now? On the main page, you will delete the Phone form field and replace it with another field called Secret Code. The 2nd page will be a continuation of the first, with a video and graphics showing them what they'll be receiving. It will also contain the rest of the order form, with shipping info, billing info and credit card, with a summary of the charge. Again, most of the code is already in place for this (as you'll see on the 3rd page if you go to my landing page referenced above), you'll just need to add the shipping fields and change the image of the order button. The 3rd page (we're eliminating one step, so it is the 4th page right now) will be the thank you page. Here I will give you some new graphics and I'll also have you pull some code from my database to pass in a unique secret code to this thank you page. We'll also have a link to send them to the registration page where the product is delivered (you'll just need to pass all of their form data forward through this link so the next site can pull it all in and prepopulate forms. Upon acceptance, I will send you scans of my drawings of what this should look like, along with all completed graphics and Infusionsoft code, as well as sample sites that have a similar look so you can examine coding. :) This needs to be completed within 1 day, so please include the words "MAGIC TODAY" in your bid or else it will be ignored.
