Change a setting on website.

Job Description:
I have a website called ModoSMS.com. I need a setting changed.
Login - [login to view URL]
Username: demo
Password: Demo
Go to Messages/Send Bulk Messages
You will see near the bottom a toggle switch that says Appended (opt-out) Message.
I need this to default to OFF. It is currently defaulted to ON.
I have included a picture and the code that were I believe this can be changed. Here is the code I think needs to be changed. (full code is attached)
