customized our own contact us page

I UNDERSTAND THERE IS LIMITATION ON 3RD PARTY TOOL (GETTING RID OF ADVERT) IF IMPOSSIBLE THEN CREATING YOUR OWN CONTACT US PAGE VIA CODING IS A MUST WITH ALL BELOW FEATURES INCLUDING “ADD FILE OR IMAGE BUTTON” AND ABLE TO ATTACH MULTIPLE IMAGE AND FILE four features I like about jotform which I am currently using which i need in new single form • i would be needing a "ADD FILE OR IMAGE" button on contact us page - **already done by jot form** • customers can attach multiple image and file at the same time if they need to.- **already done by jotform** • after customer submit form automated email is sent to customer telling them we will get back in touch within 24 hour ( please send test email to yourself to see - ** already done by jotform** • a unique code is also sent to customer email but it is too long - **already done by jotform** 1. after customer submit contact us form, a green pop up message bar appears which states “thank you for contacting us , a member of staff will get back to you within 24hour, an email has been sent to your inbox with your ticket number " (obviously in a better wording) 2. i have attached an example of how i want the contact us page ... it doesnt have to be same its just so you get the picture. (wider and bigger) i am open to your ideas too (image A) 3. after customer submit contact us form an automated email has already been sent to customer inbox by jotform. So a message should state “ thank you a member of staff will be dealing with your inquiry and will contact you within 24 hour ” or in better wording with unique ticket number (short unique id code) above I have sent you attachment of what customer would see (image B) .
