For a COV-19 Test and vaccination Center (with multiple locations), i need a Registrations Form for customers with dynamic day & time-slots (good example here: [login to view URL])
As an example. Vaccinating Days and Hours are more limited and timeslots are for Example:
Fri. 14:00 - 18:00 max 10 Persons per half an hour. While Testing slots are Mo. - Fr. 08:00 - 20:00 50 Persons/hour
But our Test/Vaccination Center has more options (has to be adjustable in backend), some options are chargeable and a option to create a bill with customers Information should be creatable.
In any way, there must be the employes Name who makes the test/vacc. Also the Name of the Vacc/test-Kit and the charge number must be selectable/writebale for the employee.
After customer fills appointment form, a confirmation email with unique code should be generated and send to customer. When customer arrives, giving code to employee, all his information is shown in backend for further processing.
This code should be reusable for coming appointments.
When customer arives at his appointment, backend should generate unique ID-QR-code for testing/vaccinating example which will be printed 2 Times to avoid confusion.
After Testing/vaccinating, the Result should be uploaded as PDF on Server, Mail should be send to customer as a link. When clicking Link, the year of birth should be Interrogate as a security feature.
