Our company uses a pdf proof approval form. The job and version # are entered on the form by the graphic designer. The end user will have a text box for describing revision requests. I want the end user to be able to submit approval or revision request using buttons at the bottom. When the user clicks the submission button, an email needs to pop up that does a few things. It needs to import data from end user and creator-filled fields and put that information in the subject line and email body. It needs to create some type of attachment (.txt or the form itself) to the email that has the same information. I don't want the end user to have to reenter any information that is already on the form into the email. You can see my work in progress attached.

Normally if a web page needs to do some JavaScript processing, this is done before the form is submitted. Once at the server, the form submission is constructed as an email which is then send to relevant parties. Anyway, I am available to complete the task you described in your bid request. You could query the form-fields value before doing submission from the button click. Adobe has a send email on button click event, but you cant control the content of email using this method. So, the better method is by writing a code for collecting all form-fields value, and then generate the mail-message with custom subject and body using javascript code, and finally do email-send. That's all.
