PHP Landing Form Page - Minor Customizations

Buget $30-250 SGD

Have a simple landing form page that has some problems that needs to be fixed.

1) Have two fields with same ID hence i cannot use the two fields in the same form. Need to allocate new id to it

2) in relation to (1), the update change will also affect the email notification that sends out

3) After filling up the form, will redirect to paypal which is already done however the email notification which contains the Paypal Transaction ID is wrong when compared. It's some random numbers that i never seen before.

4) i need to be able to change a Field from a datepicker to an input type=text field. Need guidance on it.
