update auto email

Requirements: * make the word "optional" as the default in the first name and last name fields * add a "Cell" field; the default value should be "optional" * we should be able to reference all the variables from the form in the follow up email * Ohio National PDF file names should include a "U" instead of the M or F. * add a field asking "Are you in your last year of residency / fellowship?" The drop down options should be "Yes" and "No". A 'Yes' answer searches for file names that end in "GR" while a 'No' answer searches for file names that end in 'JR". See next request to complete the idea... * All company proposals should also include "GR" for "Yes", they're in their last year. "JR" if they are not graduating this year. * please note that all pdf variables should be in ALL CAPS. The company name will continue to be in upper and lower (proper case).
