Help me build a power app in teams to automate weekly status reports

Job Description:
scope,
- design a form to allow user to enter the bullet points for the attached content (current status, plan for coming week, potential issues), form must be editable during the week, start each week with a new form
- guide me to deploy this form to teams
- at the end of each week, generate a report as attached store it in a team folder
