Fix my page that is not loading from my five9 dialer

PLEASE READ THE WHOLE PARAGRAPH Hi I use a five9 and five9 is a java program. The automated dialer that dials my customers and loads a script for me. I have 10 other agents that work for me and theirs works fine, so I think it has to be in my computer security settings, I just updated chrome 3 days ago and that is when it started acting up, if you know how to un install chrome and load the previous version would help, settings or java settings. We get a live call fill in a form, finish that form and push the data from that form into 4 other forms. My first form that is suppose to open is not responding when I push the data to it. Only 1 drop down box works and there is like 7 of them and some should already be pre filled in. Example pic attached, need done asap. Worked fine till 3 days ago and I even did a complete reinstall on my PC yesterday, but if someone else logs into my username and password and takes a call it works fine for them. So has to be a security setting on my end. Please hurry!!!!!!! Please Read entire statement.
