I need someone to create a fillable on-line form. The form needs to be sent as a link to applicant’s but i need to be able to export each response into an excel file. The responses can be in one answer form, but they can also be in paragraph format. The attached excel file shows the questions that need to be asked on the questionnaire in Row 2. I need to be able to import the applicant’s responses into Row 3. I use this document to type a report. The form can either be emailed to me, but I’d prefer if the form uploaded to my webpage and i could access it. I have a word press website
