Need a Fundraiser Page designed for Wordpress website w Giving options

I am needing a fundraiser page designed for our wordpress website ([login to view URL]). We are looking have a page created using the Give Wordpress plugin ([login to view URL]). There will need to be some customizations made. We will be wanting to provide 2 separate giving options (in separate windows:

1. A recurring giving option that defaults to donors giving $40/month for 1 year.

2. A one time donation option where the donor will specify the amount.

We will also be wanting to have a giving thermometer (available in the plugin) displayed on the page that displays the amount of funds given on both of those giving windows COMBINED.

Finally, we may also want to display the items that we are raising money for. Not sure how we would do this, but that is a thought.

I am looking for someone who is skilled in both design and customizing with CSS. Attached is the header image that we created for the fundraiser (to give you an idea of the feel we are looking for with the design).
