I need a small brochure (5x7) created. This is to be handed out and not mailed. I have attached an example that was done for me for another company, but need this one to be for the medical/healthcare industry.

Just to clarify, our company business is to identify injuries using ultrasound technology instead of using an MRI. We use ultrasound to detect muscle injuries. We don't use it to detect babies in our business.

You can take information from our website (www.echomindai.com) - the "Retail" tab is what this small brochure is advertising. This is the first retail location we are opening up.

Items that need to be included in the small brochure:

• Include the QR Code that is in the attached "Retail QR Code file"
o Include a brief description by the QR Code that explains that they need to scan the QR Code in order to schedule their appointment.
• Include the logo - versions are attached in the "Echo Mind Ai Logos" file.
• Include the address:

(located within the Dolce Vita Salon building)

• Include the phone number: (805) 695-4790
• Include the hours of operation:

M W F 9:00 AM - 6:00 PM
T TH 9:00 AM - 8:00 PM
SAT 9:00 AM - 12:00 PM

• Include the pricing: $5 per scan (normally $99)
• Include the offer timing: The promotion runs between April 3rd – 15th

Use your creativity for the design. Any other content, you can take from the website.

We will need to create one version for Doctors & Medical Staff (you can label it as such if you want) with the promotion dates of April 3rd – 15th and another for everyone else with the promotion dates of April 10th - 15th.
