I am looking for 2 business card designs. They should both be 3.5x2 inches with a 1/8 inch bleed area. They can be portrait or landscape, however, landscape is preferred.

First card - professional card:
I am looking for a simple, modern design that is suitable for letterpress printing. I would like at most 2 colours. One side can say "optometrist." or have a simple optometry related graphic of your choice. The second side should contain the following information:

Second card - appointment card
The first side should contain the attached logo as well as the information from the first card. The second side should be for filling in appointment information including: date, time and reason for appointment. Please include a note on the bottom that says "Please give at least 24 hours notice for cancellations"

Edit: A PSD file for each card would be ideal. I have included a picture of what a letterpress card looks like. The appointment card does not need to be letterpress printed.
