This project is an ASP 4.0 programming job which should take about one hour to complete. All that is required is a single ASP page that will process a handful of fields from a pre-existing form and then format and send that information to two specific email addresses.
The pre-existing form page, named [url removed, login to view] (attached), will pass information from 16 text fields by the POST method to the ASP page to be created (which must be named [url removed, login to view]). Five of those fields, marked by asterisks on the pre-existing form page, must be programmed as mandatory responses on the ASP page to be created. The ASP processing page to be created must then format the information from the form fields and pass it on via email. If the form information is processed correctly, the ASP page to be created must be programmed to then send the user to a pre-existing confirmation page (attached) called serviceSupportFormConfirmation.asp. If there is an error processing the form information, the ASP page to be created must be programmed to send the user to a pre-existing error page (attached) named serviceSupportFormError.asp.
