We had a project made on freelancer several years ago, which creates from a template, and fills in a PDF form from an online webform.
We need two mods made to it.
Add an option to a field on the PDF form
Remove the strikethrough on the PDF form.

It is a standard Appnitro Machforms webform.
Add an option to a field on the PDF form

It shows where the text entered in the webform radio buttons labelled:

It has new options added, called Set aside the judgment as per the attached order and Order for release of vehicle reg [REG] and damages pursuant to CPR 84.13 and Para 66 of Schedule 12 of the TCEA 2007 (Sch 12) due to clamping hire-purchase vehicle in breach of Para 10 of Sch 12. Costs.
These need to be added to the outputted PDF form while keeping the remaining options usable. So there should be 8 options available to the user.
2 Remove the strikethrough on the PDF form.
The attached [login to view URL] shows where the PDF form has a strikethrough, where it crosses out the words (and any continuation sheets)

The PDF form needs to be changed so that the words (and any continuation sheets) no longer have a strikethrough.
