Contact us Form with multi uploading

My current contact us form is not working properly. I need a good PHP developer who can develop a simple and fast contact us form

Your Enquiry/Comments:

Upload files if any________(Multi Uploading script allows every format, Total of files should be lesser than 25 mb, or shows error)

Verification field 0+4 = 4 (simple, take idea from [login to view URL])

SUBMIT

1. Email us the form with attachment files.

Two important things:

1. Multi uploading script should be superb(I would like to see demo first)

It Should be first attach the files and customer should see progress bar like gmail or something like that. Ajax script would be fine.

2. Then it should be emailed to us without any errors.

2. Feed this form in our database/ADMIN PANEL and make new category of QUERIES

It includes following categories.

1. Email

2. Query/Comments

3. Artwork Files

4. Status- Replied or not replied check button

5. Search through email.

6. Delete one and multiple queries.

TESTING WITH ALL MAJOR BROWSERS

REQUIRES CLEAN WORK

So, Overall it is very simple project and I would like to see demo for Uploading script you would use in the form before awarding project.
