HTML page with database and form manipulation

This project will utilise form manipulation with a database. I basically want people to recommend links for my site's blogroll. I will add the 20 most popular links to my blog.

So, what I want to do is have anyone with a valid e-mail address enter up to 20 links, and have those links stored in the database. However, the the first link they provide will be worth 20 points, and the last link worth 1 (so imagine a form with a bunch of text fields and the visitor enters their favorite sites in order of most favored to least favored). I'd like this done in PHP5/MySQL as it will reside on a LAMP server. It's a simple task, I only need three pages:

1) One page for the form itself, with 22 text fields (20 links, name and e-mail address)

2) One page for a thank you note once the user has submitted the data,

3) A simple password protected admin page that allows me to download an excel file with all the following data:

Sheet 1 Columns: Link, Total Points

Sheet 2 Columnns: Name, E-mail Address, Date/Time submitted, IP address

I would also like the text-fields to strip out "http://www." and any forward-slashes ("/") so that duplicate entries are removed. I would also like the form (using javascript/etc) to check fields so that one person doesn't enter the same link 20 times and gets 210 points).

I don't need any CSS/styling done - just plain HTML will do.
