I'm using Excel 2004.? I need to create a form that does the following:

I have a main form with a drop down. After the click of the drop down list, the form will populate the fields on the form with information from a hidden workbook.? The hidden workbook will have the database type fields to populate the form.? So the functionality is to click on an item in the drop down and the fields on the form will change to populate the fields based on the drop down item chosen.

I will need a few free form fields on the form.? The information will be saved back to the database.

I need user lables or buttons on the form that will open up another form, which is actually a hidden workbook. So the functionality is that when the user clicks on the customer label, the customer form will appear and when the user clicks on the inventory lable the inventory form will appear.

I need to create check boxes on one of the forms and the information checked need to be captured and sent to another form displayed as a list.? For instance if the user checks A, B, R and Z, then on the next form the drop down list will have only A, B, R and Z as options to choose from.

The form need the capability to color the label gray when not being used to give the appearance of thi option is not available and the label cannot be clicked when gray.

WIth all the information above being captured, I need to build an output database or fields.

One of the forms is a contact form.? I need users to complete information on the contact form and save to the database.? There will be a total of 4-5 forms.

All the information must be linked to a key field which is in the database workbook.?

custid? name,

custid custNew contact name phone city state

custid checkbox info1, checkbox info2

custid dept, dept location

The output will have 5 or 6 rows of information for one unique id
