Simple voting script

I would like a PHP-Script with a Mysql Database, where only the sum of 2 voting-possibilities are saved.

The variables are transfered via url '[url removed, login to view]' and '[url removed, login to view]' and should be saved in a mysql table. Only the Sum of the Clicks (Option 1 or Option 2) should be saved!

There should also be a result page, where the two sums of the voting should be shown.

To prevent manipulations the script should use the IP address of the user and/or Cookies. That is to make sure, every visitor votes only once. The time of the IP/Cookie-lock should be variable (default: 60 minutes).
