Modify existing PHP Script

I need a modification done.
Currently the script will allow me to import a large number of embeded, downloaded or hot linked videos using a CVS file. It provides 9 fields to use, the fields are imported via the script.
Often times I need more than 9 fields... For example one of my imports may contain 15 parts of data, but since I only have 9 fields to use, I cannot use that import file.

I need to make it so that either I have 20 or so "import fields" or the ability via ajax or something to add extra fields if needed... for the imports.. Please note the script does a "None" feature that will allow to "skip" a section of the import CSV file... hope that makes sense.
