Post Form Data to API endpoint in JSON format

I have a simple web form with approximately 8 fields that I need hooked up to process and send data to API endpoint. Using Postman, I have successfully been able to send data via an API post call but cannot get the webform to work. The data must be submitted to the API in JSON format and it will need to pass the API key in headers.
This script can be written in PHP/curl, Jquery/Ajax or any other method that will work.
Here is a look at the basic form (currently only in Html) - [login to view URL]
The Date and time field will need to be validated and out put in the following format:
○ datetime format: YYYY-MM-DDTHH:mmZ
○ validates as a future date
One other validation that must exist - The Number of Guest field will need to be an interger - guests must be a positive number greater than 0.
