I would like to create a web form for logging of personal data online that can be loaded onto the web for me to keep track of my weekly personal goals/schedule and log the data into an excel file or similar which could be stored online but needs to be downloadable for offline use. I have attached an outline of the form structure I would like.
I require the files themselves which I would then like to load onto my own website rather than creation of a website per se. (I have tried Google Apps form creator and this does not meet my requirements). Therefore, ideally I would like a simple design which is likely to be compatible with as wide a variety of web servers as possible as I have not yet decided what type of server I will be uploading to.
The attributes which I require are:
- I need multiple data entry times to be available, i.e. I would like one set of data for each week (date time stamp at top) and will be entering data at different times during the week but need this to be collated in one dataset based on the date entered at the top
- I would like to be able to edit the form labels in the future as well as the form data. This does not have to be done via a graphic UI, I am happy to be have to enter a little code if you make that obvious where it has to be done.
- The form does NOT have to be secure as I am not entering any critical personal data.
