Copy and adjust an orderform onto my wordpress installation and link it to woocommerce

I would need you to copy following orderform into my wo-commerce installation: [login to view URL]
Following fields do not need to get included: (a) Mitarbeiterzahl (Maximum, Minimum), (b) Gründungsjahr and the three Checkboxes [Telephone, E-Mail, Website].
The orderform should allow following function:
a) The client can pick and select different Industries [NACE-Branchen*] and regions [Regionen* or PLZ].
b) according to the pick, the form should calculate the maximum number of datasets available (use checked email, telephone and website for calculation) and show the total price (0.39 per dataset)
c) two fields emerge, where the client can either enter the maximum price - and the total number gets adjusted, or he can pick the maximum number - and the total price gets adjusted
d) the final number of datasets gets passed on to my woocommerce-shop
e) an email gets sent, where the selection [industry, region, numer of datasets] gets passed on to me.
