Fix Form to Work With AR Form Tags

I have a form page up.
But I need to have the Tags Fixed to Work With
my other AR Form Tags so it will work properly.
I have the form to fix.
Also the new form code tags to insert so the page form functions
correctly.
Do not change page except Captcha can be removed. Just make form work with new AR tags.
After it is fixed, we simply need to test it to make
sure correct data transmits to me correctly by prospects
who complete the form.

I would like this done immediately by someone who already
does forms, tags and can quickly make it work.
