Speech enabled forms needed

We need a program that runs mostly on Windows XP tablet editions and XP pro . The program is a data entry of a sales form, which saves to a text csv file on network share . It has a very important feature which is the form needs to be filled by speech recognition (SR) . We prefer to the program to be coded in vb or VB.NET . The program will have a lotof numeric entry ,so you have to be careful , we dont need 7 to be filled in the screen as "seven" . It is prefered too that the program can supply the network credentials to the server which has the network share .We need an advice on this part . A professional installer is required using wise or Install Shield X.
