239821 Create 4 pages in WordPress

I will be running a survey on my website, and need 2 WordPress page templates created. I will also need 2 landing pages formatted for the survey. The work will involve the following: (1) Creation of one WordPress page template. Everything will be stripped off the page except three links at the bottom (Home, About, Contact Us). In other words, the header, sidebar, etc. will be removed from the page completely. You will make the page available as a "Page Template" in my WP admin. (2) Creation of a second WordPress page template. The second page will be stripped of the sidebar, and will include only the site header and three links at the bottom (Home, About Contact Us). You will make the page available as a "Page Template" in my WP admin. (3) Creation of two landing pages for a survey. I will give you the survey content, which will include HTML code from SurveyGizmo. All I will need help with is formatting the content. The landing pages will be created as WordPress "pages" in my site (e.g. [login to view URL], [login to view URL]). I've had this kind of work done before, and don't expect it will take much time for someone experienced in WordPress. You MUST provide examples of previous work in WordPress. I won't accept any bids that don't include examples.
