PC/Mac Programing (which will reside on Flash Drive)

We want a program created that will reside on a Flash [login to view URL] program will have one version that can work on Windows 7/Vista/XP, and another that will be for Mac OS X. We will supply an program as an example of what we want. We will supply a graphical representation of the GUI illustrating the "look" that we want, and the "Sections" and "Fields" that we want the user to enter their information into. Overall Requirement of the Program We want a program made that will collect information and personal details from the user and save this data, and output an html file that can be opened and read with a browser such as Firefox or Explorer When using the program for the first time, the user will be asked to enter their information into the window that the program opens. If updating his information, he will have to click the edit button that we want in the upper Right Hand corner. This programs purpose is to collect information, and upon executing it for the first time will require information to be input into the various fields in the different sections. Once finished entering all the information, the user will exit by clicking either save or cancel. (These appear when the edit button has been clicked) The resultant data that has been collected will be saved in an html file that anyone can open using any browser, eg Firefox, safari, explorer. The initial version of this program is to be english only. The successful bidder will get follow up work, as we will want a multi language version to follow. The programmer should bear this in mind when creating the program so that the transition from single language to multi language is easier and logical. The second version will be required to switch languages when the relavent "Flag" icon is clicked upon. Details Regarding the User interface There are the following Sections on the form that the user will either see or need to fill in, and from the top of the form to the bottom in order, these are: 1. The Title Bar 2. The Header Area 3. Identification 4. Emergency Contacts 5. Allergies 6. Medical Conditions and History 7. To be split up into 3 subsections 7a. Medications 7b. Vitamins 7c. Supplements 8. Doctors 9. Insurance 10. Documentation 11. Special Instructions 12 Footer We want data entry to work just as in the example program supplied. That is the process starts by clicking the edit Button at the top of the page, For additional entry the user will click the + Plus button. For entries to be removed the - Minus button is clicked. The process of filling in the data will continue until the user has either finished filling all the data or else, has put in all the data they want to at that time. At which point they click either the save or cancel button. cancel will cause the data to revert to what it was originally and stay unchanged save will save everything as an html file viewable by and browser. Details Section by section 1. The Title Bar. We want the Title "Medic life Alert" 2. The Header. Here we want the logo, time last updated, and icons for the 4 languages that the form will eventually be available for data entry in. 3. Allergies: There is a check box next to None, giving the user the option to tick tics. The remaining fields are as can be seen in the graphic file, ie, Medication and Reaction 6. As per the graphic File, and another edit button (Or Save/Cancel depending on what the user is doing) we attach in a zip file, a sample PC version and Mac Version of similar software. And also a sample of the HTML output, and a PDF Graphical representation of the version we would like to create.
