Hey there! I am reposting this job as the freelancer I selected did not complete job to satisfaction. NOTE: You MUST provide the final file in .indd format so I can edit if required. JOB SUMMARY: Front Cover Design A5 >> opens to A4 middle spread with membership application form laying on side that takes up all room of A4 (have base design in .indd completed but will need sprucing - must be in .indd -) >> and then simple design for back cover which is text and small fields for signing a disclaimer form. If you do not understand the requirements above please do not bid. I will not release milestone unless job is exactly as requested. This is a very simple job BUT requires clever design skills to ensure front cover (small A5 size) is appealing and application form and text on back cover is laid out perfectly/neatly. ORIGINAL POSTING.... I have a simple job to design an A5 folded booklet (A4 folded portrait style with front cover - internal with application form inside) and back cover page. No internal pages - just 2 x A4 folder to create booklet style. So in total it will be Front and Back cover design and then internal fold out application form. The most time will be spent on doing the internal foldout page with form fields. I have started that in .indd but needs a little more work. Thank you!
