Create React App with a form & thank you page with text depending on chosen form values

I need a React single-page web app that I can put into my website. I will provide the UX/Graphics design. The app needs to fulfil all the following criteria: There will be two pages. The first page will have a Header Text and a simple form with 1) Two Radio Buttons, and 1) 1 Calendar Depending on values chosen from the radio buttons and the calendar, the thank you page will show different text. If the user selects a date from the radio button, the thank you page will output five lines of text with each lines having specific dates in the futures calculated from the initial date chosen by the user. Example, if a user chooses 1st Jan 2019, then one line from the thank you will say "The end date for you to contact is 30th Jan 2019 (i.e. 30 days from today". * I will need the thank you page text to be able to download as PDF. That is, by pressing a button on the page, a pdf of the page is prompted for download.. * I will need to be able to add those dates from the thank you page to user device calendar or download as csv. That is, by pressing a button on the thank you page, the dates are added to the user device calendar. Please raise any question or queries you have before starting the job. I have a hard deadline for submitting the work. The work needs to end within eight days after accepting the job. If you have never developed using React before, please do not put your bid. This should only be done using React. It is not something you can learn and do now. You will provide me with the full source code, including the webpack file. I will give you access to my bitbucket where you can pull/push code. Alternatively, you can send me your source code. If you send me a plain JavaScript/jQuery file along with HTML, I will not accept it and I will not pay you. I will only accept React files along with the HTML.
