Magento 1.6 Show Dual Pricing With Tier Pricing

We currently run a website that sells work uniforms. I want to be able to show 2 prices beside each product. The first price is the non embroidered (without company logo) price, the second is the price with embroidery (with your company logo). Example: Plain Price:£10.00 With Logo: £12.00 Every item on the website has tier pricing already set up, so i need to be able to also show the tier pricing costs for non embroidered and embroidered. A customer should be able to select if they want the embroidery (company logo) so all products form that page would add £2.00 to cover embroidery costs. Ideally this would be done in a table. I have included a screenshot of a possible solution, how it could look. I am open to suggestions on how you could do this. Applicant MUST HAVE A PROVEN TRACK RECORD IN MAGENTO - NO TIME WASTERS PLEASE!
