CRE Loaded checkout_payment page modification

Project Description: We need an expert to make the following modifications to the checkout payment page of a CRE Loaded 6.2.10 store: 1. Add 5 text areas to the [login to view URL] page: These five areas must be filled in by customer for continuing the check-out. The text in these five areas will be appended to order and included in order page. They will be included in the email sent to store owner, but not in the order notification email to customer. 2. Highlight the payment method (for example: the bar of “Credit Card ----- the radio button”) in the Payment Method area on the [login to view URL] page. When mouse rollovers this bar, the color changes.
