create a web-based form for Intuit Quickbase

Set up a web-based license request form. Developer will build the form, we will load it to our website. Form data will be written into an existing Intuit Quickbase application. We had this form in place once, and it wrote its data to Quickbase, so we know this is technically feasible. The form data is the first step in creating a new license record in Quickbase. The second step will be completed directly in Quickbase (no help is necessary for this part). please see detailed requirements document attached. ## Deliverables please see the attached document, it is very detailed, providing screen captures and mockups.
