Hello,
This project was modified and restarted with better description of the need, in order to facilitate comprehension.
I apologize for the former part.
I need 2 parts of a monitoring service frontend :
- Client Area
- User Friendly Form (html5/ajax...)
### ** The client Area :
Rather simple, it is register/authentication of the client, then access to private pages with the list of his services (don't forget reset password)
Coded in php, the auth/session part in is a file I have to include() on each page of the website, I think there already done classes for that.
Once authenticated, the only thing to do is a sample page with a list from mysql DB (SELECT * FROM hosts WHERE client='x', then show)
### ** The form, THE important part
Starting from a simple html form (attached), build a **REALLY fancy & user friendly form** to add hosts & services. (Web2.0)

After completing the form, you must insert data in the mysql tables, and provide ability to edit a host through this form.
I will provide :
- The basic HTML form, with if's, options and instructions (attached file)
- SQL schema (once bidding accepted), you may modify it if you want
- FTP access (once bidding accepted) to dev server
- Help if needed
If this project is well done by the bidder, I will have more similar developments to do, starting with a drag and drop map, others forms in client area...

## Deliverables
Here are examples of such forms/selects :

- google calendar (but very simplified and "tinyfied")
- <[login to view URL]> for the server selection layout
- [login to view URL] for the contact selection (using above layout)
