So we have to go through the site and identify nulled extension, we will first refresh core WP and update all the extension then if we have some free plugin we have go through the code of each extension for suspicious code then we review/update theme. 1) when you fix it IS all files and plugin been audited and checkd for any modifying files and code? 2) How long this hack is happenning and repeatedly? 3) Is the website in hosted or VPN or shared or dedicated? PHP/MYSQL programmer here , can review the cause and tell you for solution by doing online research for it. How often this hack is happening cos i can see he just infected all and he is gaining access if you fix one or two things. So this is an ongoing JOB so let me know from you.
