Patient demographics database build for a small healthcare clinic in Kathmandu

Good day, my name is Adam Moore. I am a nurse, currently working with a clinic in Kathmandu as a volunteer healthcare consultant. The clinic is the only place in Nepal where patients can see a doctor without payment, and also obtain medication without payment. Donations fund it. At present patient demographics are logged in a ledger, which is time-consuming, and data is not analysed. My suggestion for the clinic is that they utilise a simple database to collect and store patient information, which can be used to analyse data, i.e. identify how many new patients are male in the last month, or how many new patients attended from a particular ward. I hope that disseminating the data to stakeholders will generate more donations for the clinic. The aim of the database is to save time within the clinic, so it should be straightforward for staff to use. Later, when we have collected and stored the patient's data, it will be useful to monitor how many patients present with breathing problems, or how many new patients came in a month, for example. Ideally, it will be possible to make simple graphs etc based on the findings. A friend suggested that POWER BI would be a good tool to use. As you can probably tell, I am not a tech guy... that's where you come in. The brief is to build a database to store the patient info. The database will store patient info and have a search function, so when a patient re attends, info from their last appointments is visible. The "field names 9th aug" document provides an overview of all fields to be entered for the initial appointment and each subsequent appointment. The "presenting complaints June 29th" document is a list of the health problems a patient might present with, this will be used to populate the PRESENTING COMPLAINT field. I have grouped the problems into categories, with the heading for each category in CAPITALS. The "list of wards" document contains the drop down menu names of wards for the second address field. I intend that a patient will attend with breathing problems. The healthcare worker will click on CHEST and then select " breathing problem". This will populate the Presenting Complaint field with "Breathing Problem", and then the healthcare worker will move on to the next field, Further Information. It would be ideal if data could be stored securely on a cloud. If successful, the database will be utilised in other areas, so one small change would need to be made, just a different list of wards inputted into the address field.
