Job is a simple one. need two html forms created. first one is for 13 pieces of simple info example: name, address, phone...etc Second is a series of seven questions, some yes or no question boxes and others questions that will need simple text form fields filled out. Very easy stuff, just faster to let someone on this site handle it. winning programmer will get the html pages to plug this into, very straightforward basic html here, no rocket science. Programmer will need to make it clean and simple, css page already in place. Winning programmer will also assist in getting form mail script in place so it works properly.
